Resumen
Para evaluar el funcionamiento de una
unidad de terapia inhalatoria en un departamento de emergencia pediátrica y la
eficacia de un diagrama terapéutico fueron estudiados en forma prospectiva 200
niños menores de 2 años con síndrome broncobstructivo durante los meses de
invierno.
Los pacientes con puntaje de Tal de 5 a 8 fueron ingresados en la Unidad de
Terapia Inhalatoria, para recibir salbutamol por vía inhalatoria (2 puff cada
20 min) durante una o dos horas según la respuesta terapéutica. Se asoció
prednisona (1 mg/kg) vía oral en los niños con antecedentes de
broncobstrucción.
La estadía media en la Unidad de Terapia Inhalatoria fue de 2 horas 20 minutos;
137 niños (70%) requirieron 2 horas de tratamiento con broncodilatadores y 27
pacientes fueron además tratados con corticoides.
Fueron derivados a domicilio desde la unidad de terapia inhalatoria 54,5% de
los pacientes y desde el área general de urgencias 43% de los 77 niños con
síndrome broncobstructivo persistente.
Se concluye que la Unidad de Terapia Inhalatoria resultó eficiente para
desgravitar al Departamento de Emergencia Pediátrica de la excesiva demanda de
lactantes con síndrome broncosbstructivo en los meses de invierno y que el
tratamiento con broncodilatadores permitió derivación a domicilio al 71% de los
niños luego de una internación abreviada (54%) o de una prolongación del
tratamiento en emergencia en el 17% restante.
Summary
To evaluate the performance of a unit of inhalatory therapy of a
Pediatric Emergency Department and the efficacy of a therapeutic diagram, 200
children less than two years old with bronchoobstructive syndrome were studied
prospectively during the months of winter.
The patients with a Tal Score of 5 to 8 were admitted in the unit to receive
inhalatory salbutamol (2 puff every 20 min) during 1 or 2 hours depending on
the therapeutic response; oral prednisone (1 mg/kg) was associated to the
children with a previous history of brochoobstruction.
The average stay in the unit was 2 hours 20 minutes; 137 children (70%)
required 2 hours of treatment under brochodilators and 27 patients were
additionally treated with corticoids.
54,5% of the patients from the unit were sent to their homes and 43% of the 77
children with persistent SBOL from the urgency general area.
It is concluded that the unit was efficient to alleviate the Pediatric
Emergency Department from the excessive demand due to SBOL in the months of
winter and that the treatment with bronchodilators allowed a derivation to the
children’s homes after a short hospitalization (54%) or a continuation of the
treatment in emergency of the remaining 17%.
