Amoxicilina versus azitromicina en el
tratamiento de otitis media aguda en niños asistidos en un centro de atención
primaria

Resumen
Objetivos: 1) comparar la eficacia de
amoxicilina versus azitromicina en el tratamiento de otitis media aguda en
niños asistidos en un Centro de Atención Primaria y 2) capacitar al personal
médico de dicho Centro en el uso del otoscopio.
Método: se incluyeron 51 niños de 6 meses a 5 años, con diagnóstico de otitis
media aguda, que fueron asistidos en el Centro Materno Infantil Maciel
(Montevideo) entre junio de 1997 y enero de 1998. Los pacientes fueron
asignados en forma aleatoria a dos grupos: 1) amoxicilina 50 mg/kg/día, vía
oral, en tres dosis, por 10 días y 2) azitromicina dosis única de 10 mg/kg/día,
vía oral, por tres días. Se planificaron tres controles (día 5, 14 y 28) para
valorar la evolución clínico-otoscópica. Durante el primer mes del estudio se
realizó entrenamiento en el uso del otoscopio, supervisado por
otorrinolaringólogo.
Resultados: durante el período de entrenamiento se realizaron 32 otoscopías
coincidiendo el diagnóstico médico y otorrinolaringológico en 31 de ellas; se
diagnosticó otitis media aguda en dos. De los 51 niños incluidos en el estudio,
se comprobó la curación en 77% de los pacientes del grupo 1 y 81% de los
pacientes del grupo 2. Completaron el tratamiento 53% de los pacientes del
grupo 1 y 86% de los pacientes del grupo 2; el seguimiento fue completo en 40%
y 48% respectivamente. No se hallaron diferencias significativas en relación al
porcentaje de curación ni de fallas terapéuticas (p=0,56).
Discusión: independientemente de las fallas en el cumplimiento del tratamiento,
el porcentaje de curación en ambos grupos fue similar. Es posible que el
elevado porcentaje de curación hallado en ambos grupos sea debido a la alta
tasa de resolución espontánea en esta patología. El elevado porcentaje de
pacientes en los que no se logró completar el seguimiento refleja la dificultad
de los estudios prospectivos, que requieren seguimientos prolongados. Se
destaca la importancia de la capacitación del personal médico en el uso del
otoscopio, lo que contribuye al manejo apropiado de la otitis media aguda.
Summary
Objectives: 1) To compare the
efficacy of amoxicillin versus azithromycin in the treatment of acute otitis
media in children, in a Primary Care Center; 2) to train the medical staff of
the Center in otoscopic technique.
Methods: 51 children, 6 months to 5 years old, treated for acute otitis
media in the Centro Materno Infantil Maciel (Montevideo) between june 1997 and
january 1998, were enrolled in a prospective trial.They were randomly assigned
to one of two groups: 1) amoxicillin per os 50 mg/kg/day in 3 divided doses,
during 10 days; 2)azithromycin per os in a single dose of 10 mg/kg/day, during
3 days.Three consecutive visits were planned (at day 5, 14 and 28) for clinical
and otoscopic follow up. During the first month of the study physicians were
trained in otoscopic technique by an otolaryngologist.
Results: During the training period 32 otoscopic assessments were done;
in 31 there was agreement in the diagnosis made by physician and
otolaryngologist. Of the 51 children enrolled in the trial, acute otitis media
resolved satisfactorily in 77% and 81% of the patients in group 1 and 2
respectively. 53% of the patients in group1 and 86% in group 2 completed
treatment, and follow up was achieved in 40% and 48% respectively. Neither
resolution of acute otitis media nor therapeutic failures showed statistically
significant differences between both groups (p=0,56).
Discussion: Recovery from acute otitis media was similar in both groups,
independently of treatment compliance, probably because spontaneous resolution
is frequent in this disease. The high number of patients lost for planned
follow up visits expresses difficulties in long lasting prospective studies.
The most remarkable achievement of this study was the training of the medical
staff in otoscopic technique, which will enhance appropiate management of acute
otitis media in the future.
